Job summary

Masonicare Home Health & Hospice is seeking a dedicated Registered Nurse Case Manager to serve the Jewett City, CT area. This role focuses on delivering high-quality in-home nursing care and coordinating the plan of care for patients in the home setting.

Day shift is typical, with weekend rotation required. On-the-job training will be provided; electronic charting and OASIS data entry are part of daily responsibilities.

Qualifications

• Active RN license and current, unrestricted license.
• Minimum 1 year of clinical experience required.
• New grads will not be considered.

Responsibilities

• Provide in-home nursing care and case management for patients.
• Develop, implement and evaluate the plan of care.
• Coordinate with interdisciplinary team to address care needs.
• Supervise Licensed Practical Nurses and home health aides; conduct supervisory visits.
