Qualifications

• Access, plan, implement and evaluate individual patient care.
• Communicate effectively with patients, visitors, and the multidisciplinary team.
• Coordinate patient centred care from admission to discharge.
• Maintain accurate patient records using various clinical systems.
• Act as patients’ advocate.
• Promote the health and empowerment of patients.
• Identify changing patient needs and initiate or communicate changes.

Responsibilities

• Follow trust policies and procedures; contribute to ward/department development.
• Act as a resource link nurse for the ward.
• Develop oneself and contribute to the development of others.
• Participate in education and supervision of the ward team; act as Team/Shift leader.
