This 1099 independent contractor role requires enrollment with Medicaid as a fee-for-service provider and Home Health experience. Compensation is per assessment with mileage reimbursement and a separate No-show payment; visits occur in Hampden, Hampshire, and Franklin counties.

Qualifications
