Job summary

The People Concern is seeking a HSSP Substance Abuse Case Manager, ODR, to join the Integrated Care team at SPA 6,7,8. This full-time position supports clients in Permanent Supportive Housing, providing case management, counseling referrals, and crisis support in collaboration with licensed clinicians and peers.

Responsibilities include documenting sessions, coordinating with treatment providers, promoting harm-reduction approaches, and ensuring timely access to inpatient or intensive treatment.

Qualifications

• High School diploma or GED required.
• Certified in substance abuse counseling preferred.
• Ability to transport clients and possess a valid CA driver's license.

Responsibilities

• Provide case management and linkage to substance abuse treatment.
• Document sessions and coordinate with treatment providers.
• Collaborate with HSSP-ODR team and participate in meetings.
