Job summary

Humana is seeking a Field Service Coordinator (Care Coach 1) to assess members’ needs and guide them toward appropriate resources to promote wellness. You will meet members in their location, spend time evaluating barriers, and connect them with services to improve health outcomes.

Responsibilities include LTSS assessments, care planning, and collaboration with the care team to ensure holistic service delivery.

Qualifications

  • Bachelor’s degree in health and human services field.
  • 2+ years of related experience.
  • Proficient in Microsoft Word and Excel.
  • Excellent verbal and written communication skills.
  • Able to use a variety of electronic information applications/software.
  • TB screening required for this role.
  • Must have a valid state driver’s license and maintain personal vehicle liability insurance.
  • Must have a separate room with a locked door for home office privacy.

The Field Service Coordinator (Care Coach 1) assesses and evaluates member’s needs and requirements. This is done to achieve and/or maintain optimal wellness state by guiding members/families toward resources appropriate for their care and wellbeing. The Service Coordinator work assignments are often straightforward and of moderate complexity.

Your role will involve meeting members in their location, spending quality time assessing their needs and barriers and then connecting our members with quality services to promote their ultimate well‑being and guide health outcomes.

Responsibilities include:

  • Administer ongoing long‑term services and support (LTSS) related assessments through person‑centered thinking approaches.
  • Contacts members both telephonically and/or in‑person to establish goals and priorities. This involves evaluating resources, developing a plan of care, and identifying LTSS providers and community partnerships. The goal is to provide a combination of services and supports that best meet the needs and goals of the member and caregiver through person‑centered thinking approaches.
  • Development and modification of Service Plan and involve applicable members of the care team in care planning (Informal caregiver coach, PCP)
  • Support members through navigation of their LTSS and related environmental and social needs Use available information about member to prevent the need for administration of duplicative assessments.
  • Focus on supporting members or caregivers in accessing long‑term services and support, social, housing, educational and other services, regardless of funding sources to meet their needs.
  • Assist members in maintaining Medicaid eligibility
  • Collaborate with Medical Director/Geriatrician/Care Coordinator as deemed necessary to ensure cohesive, holistic service delivery positive member outcomes.

Required Qualifications
  • Bachelor’s degree in health and human services field
  • 2 or more years of related experience
  • Knowledge of Microsoft Office Word and Excel
  • Exceptional verbal/written communication and interpersonal skills
  • Ability to use a variety of electronic information applications/software programs
  • This role is considered patient facing and is part of Humana’s Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
  • Must be passionate about contributing to an organization focused on continuously improving consumer experiences
  • This role is part of Humana’s driver safety program and therefore requires an individual to have a valid state driver’s license and are expected to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event /$10,000 for property damage or whichever is higher
  • Must have a separate room with a locked door that can be used as a home office to ensure continuous privacy while you work
Preferred Qualifications
  • Licensed RN or Licensed Master’s Social Worker
Prior experience with Medicare & Medicaid recipients
  • Previous experience with electronic case note documentation and experienced with documenting in multiple computer applications/systems
  • Experience with health promotion, coaching and wellness
  • Knowledge of community health and social service agencies and additional community resources

Work at Home Requirements: To ensure Home or Hybrid Home/Office employees’ ability to work effectively, the self‑provided internet service of Home or Hybrid Home/Office employees must meet the following criteria: At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is required; wireless, wired cable or DSL connection is suggested. In certain roles, the minimum recommended internet speed required by Humana may not be sufficient for business needs. Humana reserves the right to require associates to upgrade their internet service if necessary. Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.

Travel: While this is a remote position, occasional travel to Humana’s offices for training or meetings may be required.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$53,700 - $72,600 per year
