Job summary

AccentCare, Inc. is seeking an RN Field Nurse for home health coverage in Reading, PA and surrounding areas. The role is PRN and paid on a per-visit basis, with an hourly compensation estimate shown in the posting.

The RN will ensure adherence to the Patient Care Plan, conduct initial and ongoing assessments, and coordinate with CHHAs and CNAs to deliver high-quality nursing care to pediatric, adult, and geriatric patients.

Qualifications

• RN with active state licensure.
• Minimum of 1 year nursing experience.
• Current CPR certification required.

Responsibilities

• Assess patients and implement individualized care plans.
• Provide direct and indirect patient care for pediatric to geriatric patients.
• Supervise and mentor CHHAs and CNAs in care tasks.
