Qualifications

• At least 1 year of experience as a nurse practitioner.
• Prescriber Drug Enforcement Agency (DEA) license must be active and unrestricted.
• Valid driver's license and reliable transportation.

Responsibilities

• Provides general medical care and care coordination.
• Performs comprehensive evaluations and establishes medical diagnoses.
• Creates and implements a medical plan of care.
