Job summary

Paradigm is seeking a Field Nurse Case Manager to cover the Sacramento County area in CA and surrounding regions. This home-based role involves travel to doctors’ offices, hospitals, and other sites to manage work-related injury cases.

You will assess, plan, coordinate, implement and evaluate care, collaborating with insurers, medical providers, attorneys, employers, and employees, while documenting outcomes for employers and carriers.

Qualifications

• Current, unencumbered RN license in California.
• Experience in workers' comp case management preferred.
• Ability to coordinate care across clinics, hospitals and insurers.

Responsibilities

• Manage medical case load for work-related injuries from assessment to evaluation.
• Coordinate with insurers, care providers, attorneys, employers and employees.
• Monitor progress and report results to employers and carriers.
• Travel to doctors' offices, hospitals and related sites as needed (up to 2 hours one-way).
