Job summary

National Care Advisors seeks a Nurse Case Manager based in Jacksonville to provide eldercare case management and coordinate with family caregivers, attorneys, and trustees. You will address clients' needs, quality of life challenges, and collaborate with a multidisciplinary team, including physicians and insurers.

The role is salaried with competitive pay, excellent benefits, and a flexible work-from-home schedule, with regional travel 1-2 times per month and occasional nationwide travel.

Qualifications

• BSN required; CCM or CRRN preferred.
• Minimum of 5 years clinical nursing experience; experience in field case management or workers’ compensation case management preferred.
• Experience with third party benefits – health insurance, Medicaid, Social Security, Developmental Disability services.
• Excellent communication skills – written and verbal.
• Proficient in using MacOS and MS Office.
• Ability to learn and adapt to new technologies in a Mac environment.
• Self-starter with strong time management and organization; comfortable in a virtual setting.
• Collaborate with team to achieve goals and resolve conflicts.
• Research and develop solutions to client challenges.
• Excellent customer service skills.

Responsibilities

NCA is in search of a Nurse Case Manager based in Jacksonville who will be responsible for case management services responsive to special needs and eldercare client care and quality of life challenges and collaboration with the primary family caregivers, attorneys, financial planners and trustees that are also serving the client.

Requirements

• Bachelor of Science in Nursing (BSN) required, CCM or CRRN preferred

• Overnight travel required regionally 1-2 times per month and occasional nationwide travel
