Adaptive Home Health is seeking a Licensed Vocational Nurse (LVN) Field Clinician to deliver skilled nursing visits in patients' homes within LVN scope under RN supervision. This role emphasizes safe, high-quality care, clear communication, and timely escalation to the clinical team.

You will support medication reconciliation, provide patient and caregiver education, coordinate with the interdisciplinary team, and help drive our technology-enabled workflow—charting and scheduling tools designed

Qualifications

• Current Colorado LVN license (unrestricted).
• Comfort working in a field-based workflow with escalation judgement.
• Strong communication and documentation skills.
• Reliable follow-through on orders, visit cadence, and patient goals.
• Current Texas driver's license and comfort with daily travel.

Responsibilities

• Deliver skilled nursing visits in the home within LVN scope, under RN/clinical supervision as required.
• Perform focused assessments, monitor changes, and escalate promptly to the RN/MD/clinical team with clear information.
• Support medication reconciliation and safety education within scope, identify discrepancies for RN review.
• Provide patient and caregiver education (teach-back) aligned to the plan of care.
• Coordinate care with the office and interdisciplinary team to ensure timely visits and order adherence.
• Document thoroughly and on time – defensible, efficient, and consistent.
