Job summary

Humana, Inc. is seeking a Field Care Manager RN 2 to assess members in their homes or community settings, coordinating care plans and services. This role requires 75–90% travel within Wayne and Macomb Counties and residence in Michigan.

The position supports complex cases, driving optimal member outcomes through interdisciplinary collaboration. The ideal candidate has an active RN/APRN license (NP/CNS) and 2+ years in health care or case management, with proficiency in EMR systems and MS Office.

Qualifications

• Must reside in Michigan (Wayne or Macomb Counties).
• Active Michigan license as RN, APRN, NP or CNS, with no disciplinary action.
• 2+ years in Health Care and/or Case Management.
• Ability to travel to homes and community settings for face-to-face assessments.
• Experience with adult population and disease management.
• Knowledge of community health and social service agencies and additional resources.
• Proficient with electronic information applications & software including EMR/EMR systems.

Responsibilities

• Manage caseload and complete assessments with members in home or community settings and by telecommunication.
• Provide clinical guidance and coordinate care plans to ensure appropriate services.
• Review and complete medical complexity attestations and oversight activities.
• Ensure services are provided in the least restrictive setting to promote well-being.
• Develop and modify Individual Care Plans with the care team involvement.
• Support members and caregivers through an interdisciplinary approach to access services.
