Job summary

VitalCaring Group is seeking a Physical Therapist to deliver evidence-based care to home health and hospice patients. In this field-based role, you will assess functional status, design and implement individualized treatment plans, and help patients regain mobility, manage pain, and safely remain at home.

You will collaborate closely with an interdisciplinary team, educate patients and caregivers, and document clinical progress.

Qualifications

• Currently licensed to practice physical therapy in Florida.
• Experience delivering care to home health or hospice patients preferred.
• Ability to design and implement individualized treatment plans and document progress.

Responsibilities

• Help patients regain mobility and manage pain so they can remain at home.
