Job summary

Elevance Health is seeking a LTSS Service Coordinator-Clinician to support members with chronic conditions through assessments, care planning, and coordination with the health team. This field-based role travels to client sites and requires weekly in-person collaboration, training, and meetings.

Candidates must hold an LPN/LVN, LSW, LCSW, LMSW, or equivalent license and have at least 2 years of experience with chronic illnesses or disabilities.

Qualifications

  • Requires LPN/LVN, LSW, LCSW, LMSW, or other non-RN license as allowed by state law and Nursing Diploma or AS in Nursing or related field, with minimum 2 years experience with chronic illnesses or disabilities in a Service Coordinator or similar role.
  • Current, unrestricted LPN/LVN, LSW, LCSW, LMSW or license other than RN in applicable state(s) required.
  • BA/BS in Health/Nursing preferred.

Responsibilities

  • Assist in identifying members for high risk complications and obtain clinical data.
  • Assist in identifying members that would benefit from alternative levels of care or waiver programs.
  • Coordinate with RN to develop, monitor, and revise member care plans to meet needs.
  • Coordinate care for members with chronic illnesses, comorbidities, and/or disabilities with health care team to ensure cost-effective use of benefits.
  • Apply clinical guidelines to member needs within licensure scope, not requiring nursing judgment.
