Job summary

UnitedHealthcare is seeking a Field Based Clinical Care Coordinator in Marion County, IN to manage complex member needs and coordinate medical, behavioral health, and social determinants of health.

The role requires 75% local travel, telecommute options within Indiana, and a Monday–Friday 8:00 am–5:00 pm schedule. Candidates need a Social Work degree (MSW or BSW with experience), MS Office skills, and a valid driver’s license.

Qualifications

• Master’s Degree in Social Work or Bachelor’s Degree in Social Work, Psychology, Special Education, Counseling, or relevant degree with 1+ year of case management experience with older adults
• 2+ years of experience in long-term support services or working with older adults
• 1+ years of experience with local behavioral health providers and community support organizations addressing SDoH (e.g., food banks, transportation, housing)
• 1+ years of experience with MS Office including Word, Excel and Outlook

Responsibilities

• Serve as primary care manager and service coordinator for high medical risk members with comorbid behavioral health needs
• Engage members face-to-face and/or by telephonic means to complete comprehensive needs assessment
• Develop individualized, person-centered care plans aligned with a member’s readiness to change
• Collaborate with internal care team, providers, and community resources to implement care plans
• Provide referrals and linkage to housing, pharmacy, and other support services
