UnitedHealth Group in Texas is seeking a Registered Nurse with community health experience for a remote role covering Travis and surrounding areas. You will assess, plan, and implement person-centered care, connect members to resources, and coordinate care across the continuum as a primary point of contact.

The position requires travel within the assigned territory up to 75% and a valid US driver\'s license.

Primary Responsibilities:

• Assess, plan, and implement person-centered strategies
• Connect members with financial, psychosocial, community, and state resources
• Coordinate care across the continuum and serve as a key point of contact
• Advocate for members and families to ensure their needs and preferences are heard and supported
• Collaborate with providers, members, caregivers, and community partners to improve health outcomes
• Travel up to 75% of the time within the Mission, TX area and surrounding areas

Required Qualifications:

• Current unrestricted Texas RN license
• 2+ years of experience in a community health or healthcare setting
• Intermediate level of proficiency with Microsoft Office applications
• High-speed internet at residence
• Access to reliable transportation and valid US driver's license with the ability to travel within assigned territory

Preferred Qualifications:

• Experience with Medicaid, LTSS, or long-term care services
• Knowledge of HCBS and integrated care principles
• Knowledge of the principles of most integrated settings, including federal and State requirements like the federal home and community-based settings regulations
• Bilingual in English and Spanish

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $29.00 to $52.00 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
