Moments Hospice in Hudson, WI seeks an RN Case Manager to lead hospice care for patients and families. You will coordinate symptom management, care planning, and interdisciplinary collaboration to ensure comfort and dignity at the end of life.
The role is full-time, field-based with a Monday–Friday schedule. A company fleet vehicle is provided for business and personal use, with coverage for gas and insurance, supporting patient visits and team leadership.
