Job summary

Vitality Hospice & Palliative Care in New Jersey seeks a compassionate Certified Home Health Aide (CHHA) for a full-time, field-based role delivering in-home care and support to patients on their end-of-life journey. Under RN supervision, you’ll assist with mobility, personal care, and companionship.

Schedule: Monday–Friday, 7:00 AM–3:00 PM, with occasional evenings or weekends. Travel required with mileage reimbursement.

Qualifications

• Active Certified Home Health Aide (CHHA) certification
• Valid driver’s license and reliable vehicle
• Current auto insurance
• Compassionate, reliable and self-directed
• Strong communication and time-management skills
• Ability to work independently and follow care plans

Responsibilities

• Assisting with mobility, transfers and ambulation
• Providing personal care (bathing, grooming, dressing, feeding)
• Offering companionship and emotional support
