Job summary

VitalCaring is seeking a Home Health RN to lead care plans, coordinate across disciplines, and drive outcomes from admission through discharge. You’ll apply advanced assessment, education, and clinical judgment to reduce hospitalizations and support patients' independence at home.

The role offers field-based autonomy within a supportive clinical team, with opportunities to impact patient outcomes, build relationships with families, and manage a dynamic caseload in the home health setting.

Qualifications

• Active RN license required (or compact) and CPR certification.
• Graduate of an accredited nursing program.
• Minimum 1 year RN experience (Home Health, Med-Surg, ICU, or Acute Care preferred).
• Strong clinical judgment and independent thinking.
• Excellent communication and patient-centered approach.
• Proficiency with EMR systems.
• Valid driver's license and reliable transportation.

Responsibilities

• Conduct comprehensive in-home assessments to evaluate clinical status and goals.
• Develop and manage individualized care plans with physicians and teams.
• Deliver skilled nursing care, including meds, treatments, and interventions.
• Serve as case manager coordinating care across therapists and aides.
• Monitor progress and adjust care based on changing conditions.
• Educate patients and families for disease management and independence.
• Coordinate transitions and discharge planning to reduce readmissions.
• Maintain timely documentation in the EMR and ensure accuracy.
• Supervise Home Health Aides as needed and participate in on-call rotation.
