Job summary

Siouxland Community Health Center is seeking an HIV Program Case Manager to deliver comprehensive case management for Part B and C patients, including intake, assessment, and care planning. The role requires field travel to rural areas and coordination with care access, housing, and support services in a busy health center.

The ideal candidate will have a Bachelor’s degree in a related field, Iowa licensure, and strong communication and data-entry skills. Bilingual Spanish is strongly preferred.

Qualifications

• Bachelor's degree in social work, psychology, or related field preferred.
• Current licensure in the State of Iowa as appropriate to degree.
• Strong computer and data entry skills.
• Travel to rural areas within the service area to provide case management services.
• Bilingual in Spanish preferred.
• Strong communication and facilitation skills in a fast-paced environment.

Responsibilities

• Provide case-management including intake, assessment, care plan development, and follow up to RW Part B and C program patients.
• Provide care coordination, linking case-management with access to care to include home visits.
• Advocate for patients regarding specialty referrals, housing and housing assessments, appointments, ADAP, transportation, financial, social services, pharmaceutical companies and support groups.
• Perform HIV and Hepatitis C testing, i.e., “Free and Confidential Testing,” both in-house and in outreach.
• Other duties and projects as assigned.
