Qualifications

  • Master's Degree with 1–2 years experience in care coordination or related field.
  • Bachelor's Degree with 3–5 years experience in similar roles.
  • High School diploma with 5–7 years experience.

Responsibilities

  • Daily travel within KS counties Leavenworth, Wyandotte, Douglas, Johnson, Franklin, and Miami.
  • Support population health initiatives for assigned care team(s).
  • Support functions of the care team required to manage the assigned population.
  • Process inpatient and outpatient prior authorization requests.
