Job summary

The Bridge Inc. is seeking a Neighborhood Navigator Primary Care Nurse Practitioner to deliver compassionate, trauma-informed primary care in community and field settings to adults with serious mental illness and substance use disorders.

This field-based role emphasizes care coordination, MAT prescribing for opioid use disorder, and collaboration with housing providers and other health professionals across Manhattan neighborhoods.

Qualifications

• Current NY Nurse Practitioner license or eligibility
• National board certification in NP specialty
• Experience delivering primary care in community/behavioral health settings

Responsibilities

• Provide primary and basic medical care in community-based and field settings
• Conduct comprehensive medical assessments and ongoing health monitoring
• Diagnose, treat, and manage acute and chronic conditions within NP scope
• Prescribe and manage medications, including MAT for opioid use disorder
• Coordinate referrals to primary care, specialty providers, and community resources
• Collaborate with housing providers, shelter staff, hospitals, and treatment partners
• Participate in interdisciplinary team meetings and care coordination activities
• Maintain accurate EHR documentation and required administrative reports
