Job summary

VitalCaring Group in Basile, LA is seeking a Sales Care Transition Navigator (Home Health) to coordinate hospital-to-home transitions and ensure seamless patient care.

You will collaborate with hospital teams, clinicians, patients, and families to reduce readmissions, while managing discharge planning and care coordination. A field-based role requires strong communication and organization to support agency growth.

Qualifications

• Active RN, LVN/LPN, or PT license in the state of employment (or compact eligibility).
• Minimum of two (2) years of clinical experience; home health or post-acute experience preferred.
• Experience in healthcare coordination, case management, clinical care, or hospital-based roles.
• Strong understanding of patient care transitions, discharge planning, or post-acute services.

Responsibilities

• Serve as primary liaison between hospital teams, patients, and VitalCaring clinicians for hospital-to-home transitions.
• Conduct bedside assessments to identify needs and barriers to discharge.
• Collaborate with case managers and physicians to develop patient-centered transition plans.
• Coordinate referrals to drive timely admissions and smooth handoffs to home health services.
• Build trusted relationships with hospital partners through consistent communication.
