Job summary

18 Degrees is seeking a Family Support Worker to perform home visits for families in Hampden, Hampshire, and Franklin Counties, referred by the Department of Children and Families. The role includes supervising Family Time Visitation and coordinating services to support family goals.

The position requires strong collaboration, documentation, and dependable transportation, with occasional evenings or Saturdays and a preferred bachelor’s degree or equivalent experience.

Qualifications

• Bachelor's degree preferred or 5 years of relevant experience.
• Occasional evenings or Saturday availability required.
• EOHHS CORI, National SORI, Driving Record Authorization and DCF Background Check must be completed.

Responsibilities

• Provide home visits for families referred by the Department of Children and Families.
• Facilitate Family Time Supervised Visitation referred by DCF, Family and Probate Court, and Post Adoption.
• Coordinate needed services, offer warm referrals, advocacy, and documentation of progress.
• Build trusting relationships while maintaining appropriate professional boundaries.

This full-time position at our West Springfield location is for a Family Support Worker providing home visits for families in Hampden, Hampshire and Franklin Counties who have been referred by the Department of Children and Families. This position also includes facilitating Family Time Supervised Visitation referred by the Department of Children and Families, Family and Probate Court, and Post Adoption.

Pay Rate: $22.02

• A knowledge of strength-based engagement with at-risk children and families. You’re able to support families in identifying their goals & strengths, track and document progress; coordinate needed services; provide warm referrals, individualized support & advocacy
• Skills in collaboration and communication
• Solid understanding of community-based resources
• The ability to build trusting relationships while maintaining appropriate boundaries
• Dependable transportation
• Excellent organizational skills and good writing/computer skills

Preferred Experience:

• Experience working with Transitional-Aged Youth
• Lived experience in Recovery

Qualifications:
