Qualifications

• Bachelor’s degree in a social services related field or equivalent.
• Bachelor’s or Associate degree in a human services field (or equivalent).
• High school diploma/GED plus 5 years of full‑time social services experience.
• Must be at least 23 years old.
• Strong interpersonal and communication skills.
• Effective problem solving and coordination abilities.
• Basic MS Office skills.

Responsibilities

• Conduct home visits and provide individualized support to families.
• Assess family needs, set goals, and connect with community resources.
• Advocate for families and help navigate community systems.
• Maintain timely documentation of contacts, services, and progress.
• Support family progression and participate in discharge planning.
