Job summary

Health Connect America, Inc. in Savannah, TN is seeking a Family Support Specialist to deliver best-practice, quality clinical services to children, families, and adults, following COA and state standards.

You will develop treatment plans with the team, provide individual, group, and family interventions, ensure timely documentation, and participate in weekly Treatment Team meetings while recruiting new referrals and maintaining compliance.

Qualifications

• Bachelor's degree in a human services discipline or BA/BS with 3 years of relevant experience.
• 1 year of experience working with children preferred (internships or volunteer acceptable).

Responsibilities

• Communicate with customers, colleagues, and other individuals to answer questions, disseminate, or explain information, and address complaints/concerns.
• Develop treatment plans with the input of the treatment team and the Clinical Supervisor
• Provide individual, group, and family therapeutic interventions and skill building as outlined in the Treatment Plan
• Implement Health Connect America approved therapeutic interventions as delineated on the treatment plan with clients and their families
• Complete all client documentation within established time frames
• Attend and actively participate in weekly Treatment Team meetings
• Link the client and family with specific services and resources as identified in the treatment plan
• Serve as a liaison with other professionals and agencies as related to the client
• Actively recruit new referrals to ensure that your caseload stays full
• Provide 24/7 on-call support for clients as per program requirements
• Ensure compliance with all state regulatory bodies & COA standards.
• Participate in and complete all assigned training and professional development activities, including E Learning courses.

Qualifications

• A Bachelor's degree in a human services discipline such as Social Work, Psychology, Sociology, Counseling, or Criminal Justice, along with 1 year of experience working with children (including internships or volunteer experiences) is preferred, OR a BA/BS degree with 3 years of relevant experience.
