Champions for Children, Inc. is seeking a Part-Time Family Specialist to support health, wellness, and education for families in pediatric care.

The role involves building trusting relationships during well-child visits, coordinating referrals, and connecting families with resources through various contact methods. The position requires a high school diploma or equivalent, with preferred associate degree and CHW eligibility.

Qualifications

  • High School diploma or equivalent; associate degree preferred.
  • Experience in community-based setting with young children and families (1–2 years preferred).
  • Bilingual English/Spanish or English/Creole highly valued.
  • CHW certification eligibility. Proficiency with Microsoft 365 suite.

Responsibilities

  • Establish trusting relationships with patients and families during visits and follow-ups.
  • Provide parent/newborn support via phone, email, or community visits.
  • Coordinate referrals to address unmet social needs and connect to resources.
  • Enter data into EHR and participate in quality improvement activities.
  • Attend weekly interdisciplinary team case reviews and related trainings.

From prenatal through school-age, our evidence-based, nationally accredited family education fosters healthy child development by enhancing practical skills, confidence, and connections to essential resources.Our MissionChampions for Children’s mission is to support healthy child development by empowering families through education, coaching, and strong family and community connections.Our VisionChildren are safe, loved, and surrounded by strong, supportive families.PROGRAM/DEPARTMENT: DULCETITLE: Family SpecialistCLASSIFICATION: Part-Time (25 hours per week), non-exempt.SUMMARY: Family Specialists support health, wellness, and education for families in pediatric care. They build trusting relationships with families during well-child and acute care visits, and maintain contact through phone, text, email, or home visits. Using evidence-based education and nationally approved guidelines, they help families care for their children and follow care plans. They also connect families to resources for unmet economic, social, and legal needs. Their responsibilities include data collection, patient documentation in the EHR, and participating in quality improvement activities and training. This family-facing role reports to the program manager.ESSENTIAL RESPONSIBILITIES, DUTIES, AND TASKS:Responsible for establishing trusting relationships with patients and their families.Meet with families at routine Well Child Visits and telephone check-ins, supporting the parent-child relationship, providing information on child development, and supporting unmet social needs by coordinating referrals for concrete supports.Provide ongoing parent/newborn support through phone, test, email, and community visits with a focus on parental and cultural strengths.Continually assess the need for additional resources for the family, especially relating to developmental concerns by building strong, honest, trusting partnerships with families that can address concerns in a supportive manner.Collaborate with the legal partners to ensure families are provided concrete supports.Administer screening tools to ensure that families are supported in caring for their infants and adhering to the recommended plan of care.Always maintain HIPAA and confidentiality compliance, strictly adhere to policies which ensure participant confidentiality.Exhibit excellent working relationships with patients, visitors, colleagues, and facility personnel.Engage in individual reflective supervision sessions.Timely data entry into participant records and data collection systems.Prepare for and participate in weekly case reviews with the Interdisciplinary Team.Participate in quality improvement activities.Participate in outreach and community events.Participate in related training and workshops.Perform other related duties as assigned or requested.Champions for Children, Inc. is an equal opportunity employer. New employees accrue PTO with each pay period at a rate equivalent to 15 days per year. PTO benefits increase on the 2nd, 5th, and 10th work anniversaries.
