Qualifications

• Bachelor’s degree in social work or related Human Services field from an accredited institution is required.
• Minimum of one year experience working with children in a full-time position.
• Minimum of one year experience providing strengths-based case management services through a trauma-responsive lens.
• Knowledge of community resources, service referrals, and linkage.
• Excellent leadership skills and customer service approach with discretion for sensitive information.
• Strong organizational skills with attention to detail and ability to multitask.
• Ability to de-escalate crises with tact and maturity.
• CPR and First Aid certification required.
• Maintain a safe driving record.

Responsibilities

• Conduct initial intake assessments of families to identify needs, strengths, and goals.
• Formulate service plans addressing identified areas of need.
• Establish and maintain positive relationships with youth and caregivers.
• Visit program sites weekly to check in with youth and caregivers.
• Conduct case coordination through wraparound processes involving youth and families.
• Connect families with community resources and referrals.
• Develop relationships with community organizations to facilitate access to resources.
• Monitor and evaluate service effectiveness and adjust as needed.
• Assist families with Flex Fund applications and coordinate crisis intervention services if needed.
• Provide ongoing support and guidance throughout participation in the program.
• Conduct regular follow-up meetings to track progress and adjust plans.
• Advocate for youth and caregivers with external agencies.
• Review and approve program documents, assessments, service plans, and progress notes.
• Maintain confidentiality per policies and regulations; obtain necessary releases.
• Prepare and submit regular reports on client progress and program outcomes.
• Ensure compliance with program policies and regulatory requirements.
