Family Ark, a community-based organization in southern Indiana, seeks a Community-Based Case Manager to provide in-home case management and collaborate with families, schools, and professionals to reach family goals.

The role involves developing treatment plans, submitting monthly reports to the Department of Children's Services, and participating in case conferences and court proceedings as needed. It requires strong communication and the ability to work with diverse populations.

Qualifications

• Bachelor's degree in social work, psychology, sociology, or a directly related human service field preferred.
• One year work or internship experience in the social service field.
• Strong verbal and written communication skills.
• Ability to interact effectively with people from diverse social and economic backgrounds.
• Candidates are subject to pre-employment drug/alcohol testing as Family Ark is a drug free workplace.

• Maintains all client case records by documenting contacts with families, schools, parents, and other professionals.
• Develops treatment plans, revises treatment plans and submits monthly reports to the Department of Children's Services.

• Participates in a 24-hour on-call rotation and provides support as needed.

Job Description

Family Ark provides case management services to youth and families in southern Indiana. The Community-Based Case Manager provides case management services in the home and collaborates with team members, schools, families and other professionals to meet each family's specific goals and needs. This individual works with DCS-involved families who have had a substantiated incident of abuse and/or neglect.

Key Responsibilities Include

• Facilitates case management services primarily in the home of the client.
• Maintains all client case records by documenting client contacts, as well as contacts with the Department of Children's Services, schools, parents, and other professionals.
• Develops treatment plans, revises treatment plans and writes and submits monthly reports to the Department of Children's Services.
• Participates in case conferences and court proceedings as necessary for client care.
• Participates in a 24-hour on-call rotation and provides support as needed.
• Other duties as assigned.

Minimum Requirements

• Bachelor's degree in social work, psychology, sociology, or a directly related human service field preferred.
