Job summary

Muslim Community & Health Center in Milwaukee, WI seeks a Family Practice Physician to deliver comprehensive, patient-centered primary care for individuals and families across the lifespan within an FQHC Look-Alike setting.

You will work with an interdisciplinary team to provide preventive care, manage chronic conditions such as diabetes and hypertension, and coordinate care with behavioral health, dental, and specialty services while using an EHR to document care.

Qualifications

• MD or DO from an accredited medical school.
• Completion of an accredited Family Medicine residency program.
• Board certified or board eligible in Family Medicine.
• Active and unrestricted medical license in WI.
• DEA certification.
• Basic Life Support (BLS) certification.

Responsibilities

• Provide full-spectrum primary care services, including diagnosis, treatment, and management of acute and chronic conditions for patients of all genders and ages.
• Deliver preventive care services, including annual exams, immunizations, screenings, and health education.
• Manage chronic diseases such as diabetes, hypertension, and asthma using evidence-based guidelines.
• Perform routine in-office procedures.
• Provide women’s health services, including contraceptive counseling.
• Coordinate care with behavioral health, dental, and specialty providers as part of an integrated care model.
• Utilize electronic health records (EHR) to document patient encounters accurately and timely.
• Participate in care team huddles, case conferences, and quality improvement initiatives.
• Support population health efforts, including outreach and care gap closure.
• Provide culturally competent, trauma-informed care to a diverse patient population.
• Participate in after-hours call rotation as needed.

Position Summary

The Family Practice Physician provides comprehensive, patient-centered primary care services to individuals and families across the lifespan within a Federally Qualified Health Center Look-Alike (FQHC Look-Alike) setting. This role is dedicated to serving a diverse and often underserved population, with a strong emphasis on preventive care, chronic disease management, and whole-person health. The Physician works collaboratively within an interdisciplinary care team to deliver high-quality, culturally responsive care aligned with FQHC Look-Alike mission and values.

Minimum Qualifications

• MD or DO from an accredited medical school
• Completion of an accredited Family Medicine residency program
• Board certified or board eligible in Family Medicine
• Active and unrestricted medical license in WI

Preferred Qualifications

• Prior experience working in an FQHC, community health center, or underserved setting
• Bilingual skills
• Experience with integrated care models (behavioral health, dental coordination)
• Familiarity with value-based care and quality metrics
