Qualifications

• D.O. or M.D. required and board-certified.
• Completed residency in primary care; outpatient experience preferred.
• Illinois licensure, DEA and CME requirements.
• EPIC EHR proficiency desired.

Responsibilities

• Provide full scope of primary care duties including diagnosis and treatment.
• Coordinate care and manage patient follow-up and referrals.
• Document visits within 24 hours and respond to messages within 72 hours.
• Fulfill on-call responsibilities as scheduled and maintain quality standards.
• Prescribe medications per Illinois statutes and guidelines.
