Easterseals NH & VT is hiring a Family Outreach Worker for our Residential Educational Services program serving the Manchester area. You will provide case management to youth in residence and their families, coordinating with social services and the clinical team to support treatment goals.

You will ensure medical appointments are current, communicate with families and collaterals, and maintain compliant documentation.

Qualifications

• A minimum of a Bachelor’s Degree in Human Services or related field of study.
• Two years of experience working with children and families.
• A valid driver’s license, reliable transportation, Auto Insurance.

Responsabilités

• Provide case management services to youth in our residential program and to their families.
• Collaborate with social services to provide family-centered practices in the community.
• Assess family resources and identify strategies to support youth and family.
• Work with the residential and clinical teams to support treatment goals.
• Ensure all medical appointments are up to date and communicate with families and collaterals.
• Maintain compliant documentation including authorizations and reporting.
