This role emphasizes value-based care, preventive medicine, and coordinated care within a collaborative multidisciplinary team. The position offers a 4-day work week, a $30,000 sign-on bonus, relocation assistance, and a competitive salary range with extensive benefits including

Qualifications

• MD or DO from an accredited medical school.
• Active Arizona medical license (or eligibility to obtain).
• Current DEA registration.
• Board Certified or Board Eligible in Family Medicine or Internal Medicine.
• Experience caring for geriatric, post-acute care, SNF, LTC, or medically complex patient populations is preferred.
• Excellent clinical judgment, communication, and team collaboration skills.
• Commitment to delivering compassionate, patient-centered care.
