Job summary

Community Health Programs (CHP) seeks a Family Nurse Practitioner to provide advanced primary care within an FQHC network serving rural Berkshire communities. The NP will conduct assessments, develop care plans, and lead patient care with a target of seeing multiple patients per hour.

The role requires MA licensure, ANCC/AANP certification, prescriptive authority, and a solid background in primary care nursing, HIPAA, and data management. CHP emphasizes access, quality care, and health equity.

Qualifications

• Master's degree from an accredited nursing program.
• Massachusetts license and board certification (ANCC or AANP).
• Prescriptive authority and FNP primary care experience.
• Knowledge of data management and medical records systems.
• Familiarity with HIPAA and OSHA regulations.

Responsibilities

• Perform physical exams and patient observations.
• Record medical histories and symptoms.
• Create care plans and contribute to existing ones.
• Order, administer, and analyze diagnostic tests.
