You will work with a team of MAs, care coordinators and clinical pharmacy support to close care gaps and improve outcomes, supported by a value-based model and modern EMR tools.

Qualifications

• Master of Science degree in Nursing from an accredited program.
• ANCC or AANP Certification.
• Meets ASAS credentialing standards.
