Community Health Organization in Northern California is seeking a Family Nurse Practitioner to provide generalized primary care across all ages in a community health setting, with sites in Yuba City, Chico, and Los Molinos, near San Francisco.

You will collaborate with physicians and the health team to perform histories, exams, and screening; order labs and imaging; manage ongoing health maintenance; and refer complex cases to physicians as needed.

Qualifikationen

• Valid California RN license.
• FNP certificate from a program of at least 18 months (12 months didactic and clinical work + 6 months supervised clinical internship).
• Current CPR certification.
• Effective communication and time management skills.
• Ability to interact with patients and the public courteously and calmly.

Aufgaben

• Provide generalized health care services in a community health setting.
• Take principal responsibility for primary health maintenance of patients of all ages.
• Take medical histories, perform examinations, and initiate screening and diagnostic procedures.
• Initiate therapeutic interventions and explain disease processes to patients.
• Manage ongoing health maintenance and clinical care, including counseling.
• Interpret and review laboratory tests and medication refills, and initiate orders for labs and x-rays.
• Determine cases to refer to the physician for further diagnosis and treatment.
• Serve as a clinical consultant to nursing and paramedical personnel.
• Coordinate with families and community agencies with knowledge of community resources.
