• Experience in independent family practice including wellness care and chronic care management.
• CPR/BLS certification required at time of start date.

Responsibilities

• Practice relationship-based medicine at the top of your license.
• Offer wholistic care including prevention, chronic disease management, and health education.
• Work autonomously while prioritizing patients and utilizing available resources.
