Job summary

Qualifications

• Board Certified or Board Eligible Family Nurse Practitioner.
• National certification as a Family Nurse Practitioner through an accredited certifying board.
• Licensed or eligible for APRN licensure in Vermont.
• Must be committed to providing compassionate, high-quality care to diverse and underserved communities.

Responsibilities

• Outpatient-only role with no obstetric, inpatient, or call responsibilities.
• Collaborative environment in a Patient-Centered Medical Home model focused on evidence-based care, access, and coordination.
• Manage an average workload of around 16 patients per day with focus on quality care.
