Job summary

Godwins Family Care LLC is seeking a part-time Family Nurse Practitioner for a hybrid role based in Savannah, GA. You will perform comprehensive primary care, including health assessments, diagnosis, and management of acute and chronic conditions in home-based and clinic settings, with some work-from-home duties.

You will collaborate with physicians, other advanced practitioners, care coordinators, and community resources, documenting encounters in the EHR, participating in remote monitoring

Qualifications

• Current Georgia Family Nurse Practitioner licensure and national board certification (FNP-BC or FNP-C).
• Experience in primary care, family practice, or home-based care with assessment, diagnosis, and chronic disease management.
• Strong communication, education, and counseling skills with diverse populations.
• Proficiency with EHRs, telehealth/remote monitoring tools, and clinical documentation.

Responsibilities

• Provide primary care services including health assessments, diagnosis, and treatment planning.
• Manage ongoing care for acute and chronic conditions in home-based and hybrid settings.
• Review medical histories, order and interpret diagnostics, and prescribe medications.
• Deliver preventive care and patient education to individuals and families.
• Collaborate with physicians, care coordinators, and community resources to ensure care continuity and transitions.

Godwins Family Care LLC is a physician- and family nurse practitioner-owned healthcare organization serving nine counties across Metro Atlanta and surrounding areas. The organization provides private home care that supports aging adults with clinical oversight, dementia and behavioral specialty services, and consistent caregivers. Its Continuous Care Program offers 24/7 remote or hybrid monitoring, enabling early identification of concerns, proactive communication, and direct coordination with clients’ medical teams. In-home primary care brings advanced practitioners into patients’ homes, emphasizing prevention and access to high-quality care. Godwins Family Care also conducts independent medical examinations for insurers, attorneys, and government agencies, all grounded in clinician-led oversight and trusted, patient-centered care.

Role Description

This part-time Family Nurse Practitioner role is a hybrid position based in Savannah, GA, with a mix of in-person community and home visits and some work-from-home responsibilities. The practitioner will provide comprehensive primary care services, including health assessments, diagnosis, treatment planning, and ongoing management of acute and chronic conditions in home-based and hybrid care settings. Day-to-day tasks include conducting patient visits, reviewing medical histories, ordering and interpreting diagnostic tests, prescribing medications, and delivering preventive care and patient education. The role involves collaborating closely with physicians, other advanced practitioners, care coordinators, and community resources to ensure continuity of care and effective care transitions. The practitioner will document encounters in the electronic health record, participate in remote monitoring workflows, and support quality improvement initiatives and evidence-based clinical protocols.

Qualifications

• Current, unrestricted licensure as a Family Nurse Practitioner in the state of Georgia and national board certification (FNP-BC or FNP-C).
• Demonstrated experience in primary care, family practice, or home-based care, including assessment, diagnosis, treatment, and chronic disease management.
• Strong skills in patient communication, education, and counseling, with the ability to build trust and work effectively with diverse populations and families.
• Proficiency with electronic health records, telehealth or remote monitoring tools, and accurate clinical documentation.
• Ability to work independently in the field, manage a flexible schedule, and prioritize patient visits and follow-up tasks in a hybrid work environment.
• Comfort with interdisciplinary collaboration and care coordination among physicians, specialists, caregivers, insurers, and legal or government entities.
• Valid driver’s license, reliable transportation for in-home visits, and capacity to travel within the Savannah, GA area as needed.
• Master’s degree in Nursing from an accredited program; prior experience with geriatric care, dementia care, or behavioral health is highly beneficial.
