RRC Solutions Group is seeking a Family Nurse Practitioner for 100% outpatient primary care across Long Island locations. Flexible hours, full-time or part-time, with no on-call requirements. The role focuses on preventive, acute, and chronic care, working with a multidisciplinary team.

The ideal candidate has NYS RN/NP licensure, BSN/MSN, 8+ years of NP experience, and strong communication skills to lead patient care and population health initiatives.

Formación

• RN/NP licensure in NY and board certification as an NP.

Responsabilidades

• Provide high-quality outpatient primary care including preventive and chronic disease management.
• Serve as a key clinical provider within the practice and coordinate care.
• Refer patients to specialty services as needed and document clearly in EMR.
• Collaborate with physicians, specialists and care teams to ensure continuity of care.
• Provide clinical oversight to staff and ensure HIPAA and applicable regulations.
• Participate in population health initiatives and quality improvement projects.
• Develop sustainable patient panels aligned with growth objectives.

Family Nurse Practitioner

Compensation: $200K-$300K per year

Job Type: Full time or Pat time. Flexible hours, Monday–Friday, No call.

A well-established, physician-led group with locations throughout Long Island, is currently seeking to hire a Family Nurse Practitioner. This practice has built a stellar reputation for clinical excellence, patient satisfaction, and multidisciplinary collaboration. 100% outpatient.

Key Responsibilities:

• Serve as a key clinical provider in the delivery of high-quality care.
• Provide direct patient care in an outpatient primary care setting, including preventive, acute, and chronic disease management.
• Refer patients to appropriate specialty services as needed.
• Provide clear communication and education to patients regarding diagnoses, treatment options, and preventive strategies.
• Maintain thorough and accurate documentation within the Electronic Medical Record (EMR) system.
• Collaborate with referring physicians, specialists, and care teams to ensure seamless patient care.
• Provide clinical oversight to nurses, technologists, and support staff within the practice.
• Stay compliant with HIPAA, NYS DOH, CMS, and relevant payer guidelines.
• Participate in population health initiatives and support ongoing quality improvement projects.
• Contribute to the development of a sustainable patient panel aligned with practice growth objectives.

Qualifications:

• BSN and MSN from an accredited nursing school
• NYS RN & NP license, BLS, Board Certified
• Strong diagnostic, communication, and interpersonal skills.
• Team-oriented with a patient-centered approach to care.
• Proven ability to maintain and grow a local clinical practice is required
• Entrepreneurial mindset with an ability to grow and manage a thriving patient base.
• Familiarity with EMRs and value-based care metrics is a plus
• 8+ years experience as a NP
