Job summary

Jane Pauley Community Health Center in Indianapolis, IN, is seeking a Family Nurse Practitioner (FNP) to deliver full-scope primary care within an FQHC model. Qualifications

  • Master’s or Doctoral degree in Nursing from an accredited program.
  • Current, unrestricted Indiana Nurse Practitioner license.
  • Board certification as a Family Nurse Practitioner (FNP).
  • Prescriptive authority and DEA licensure (or ability to obtain).
  • Strong clinical assessment, diagnostic, and decision-making skills.
  • Excellent communication and collaboration skills.

Responsibilities

  • Conduct patient assessments, physical exams, and diagnostic evaluations.
  • Diagnose and manage acute and chronic conditions across the lifespan.
  • Develop and implement individualized treatment plans.
  • Prescribe medications and perform procedures within scope of practice.
  • Provide preventive care, screenings, and health maintenance services.
  • Maintain accurate documentation in the EHR and ensure HIPAA compliance.

Position Overview

The Family Nurse Practitioner provides comprehensive primary care services; focusing on prevention, diagnosis, and management of acute and chronic conditions.

This role partners with physicians and interdisciplinary teams to deliver high-quality, coordinated care; supporting organizational goals related to access, quality, and patient experience.

What You’ll Do
Clinical Care & Patient Services
  • Conduct patient assessments, physical exams, and diagnostic evaluations
  • Diagnose and manage acute and chronic conditions across the lifespan
  • Develop and implement individualized treatment plans
  • Prescribe medications and perform procedures within scope of practice
  • Provide preventive care, screenings, and health maintenance services
Patient Education & Care Coordination
  • Educate patients and families on disease management, prevention, and wellness
  • Collaborate with care teams to ensure coordinated, patient-centered care
  • Support transitions of care and appropriate referrals within and outside the network
Quality & Population Health
  • Participate in quality improvement initiatives and care gap closure efforts
  • Support population health strategies and value-based care programs
  • Monitor patient outcomes and contribute to continuous improvement efforts
Documentation & Compliance
  • Maintain accurate and timely documentation within the Electronic Health Record (EHR)
  • Ensure compliance with HIPAA, regulatory, and organizational standards
  • Support coding, documentation accuracy, and billing requirements
Required
What You Bring
  • Master’s or Doctoral degree in Nursing from an accredited program
  • Current, unrestricted Indiana Nurse Practitioner license
  • Board certification as a Family Nurse Practitioner (FNP)
  • Prescriptive authority and DEA licensure (or ability to obtain)
  • Strong clinical assessment, diagnostic, and decision-making skills
  • Excellent communication and collaboration skills
Preferred
  • Experience in primary care, community health, or FQHC settings
  • Experience working with diverse or underserved populations
  • Familiarity with EHR systems such as Epic
  • Experience in team-based or integrated care environments
