Job summary

Advantage Care Health Centers is seeking a Family Nurse Practitioner to deliver primary medical care to adults and children under physician supervision in our Glen Head facility. You will conduct exams, order and interpret tests, develop care plans, and coordinate with community resources to ensure comprehensive patient care.

The role requires NY licensure, a Master's in Nursing, CPR/AED, and strong communication. Preference for bilingual abilities and EMR experience.

Qualifications

• Master's degree in Nursing is required.
• Excellent verbal and written communication skills are essential.
• Bi-lingual (English/Spanish) preferred.
• New York State licensure is required.
• 28 Continuing Medical Education (CME) credits every credentialing cycle are expected.
• Healthcare Provider CPR/AED certification is required.
• Knowledge of EMR, Medicare/Medicaid and third-party billing is preferred.
• Familiarity with federal and state regulatory requirements.

Overview

The Family Nurse Practitioner is responsible for the daily activities of the health care facility related to patient primary medical care and treatment under the direction of the CMO. Coordinates and oversees the provision of medical services to adults and children. Ensures the provision of in-service education to staff. Works with the CMO and CEO to ensure provision of quality care and services.

Responsibilities

• Provides general primary medical care and treatment to adults and children in the health care facility under the direction of a physician
• Performs physical examinations and preventive health measures within prescribed guidelines and instructions of physician
• Orders, interprets, and evaluates diagnostic tests to identify and assess patient\'s clinical problems and health care needs
• Records physical findings, and formulates plan and prognosis, based on patient\'s condition
• Coordinates services of the center with the services of community health facilities and programs, and community social agencies
• Prescribes or recommends drugs or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures
• Refer patients to physician for consultation or to specialized health resources for treatment
• Provides oversight and direction in scheduling follow-up patient care
• Participates in the oversight and direction regarding appropriate chart documentation through regular interaction and communication with medical providers; participates in establishing procedures to optimize the handling of medical records and related documents, including development of forms and reports
• Conducts wellness visits
• Develops goal setting and barrier resolution techniques to assist participants in developing individualized plans for successful behavior change
• Develops and implements patient care plans (PCMH and CCM), instructs and counsels patients and records progress towards goals
• Collaborates with the CMO to develop and/or coordinate the provision of in-service education to all health care center staff, as indicated
• Ensures compliance with social distancing, PPE usage, and infection control protocols

Qualifications

• Nursing degree conferred by an accredited college or university; Master\'s level required
• New York State licensure required
• 24 Continuing Medical Education (CME) credits per credentialing cycle (2 years)
• Healthcare Provider CPR/AED Certification
• Three years of experience treating persons with intellectual and developmental delays or disabilities preferred
• Knowledge of Electronic Medical Record (EMR) preferred
• Knowledge of Medicare, Medicaid and third-party billing requirements preferred
• Knowledge of relevant federal and state regulatory requirements required
• Excellent verbal and written communication skills, including strong organizational and interpersonal skills
• Proficient in computer applications (e.g., Microsoft Office Word, Outlook)
• Bi-lingual preferred (English/Spanish)
