Job summary

Southeast Healthcare, LLC is seeking a Family Nurse Practitioner to join our FQHC team serving rural Ohio communities. You will deliver comprehensive primary care across all ages, conduct exams, diagnose and treat illnesses, and manage MAT where appropriate.

You will order tests, provide preventive care, and coordinate with specialists and community resources. Ideal candidates hold an Ohio FNP license and DEA registration, with experience in family practice or an FQHC setting.

Qualifications

• Current licensure as a Family Nurse Practitioner in the State of Ohio.
• Current DEA registration and prescriptive authority.
• Experience in family practice, community health, or an FQHC setting preferred.
• Strong clinical assessment, communication, and interpersonal skills.

Responsibilities

• Provide comprehensive primary care services to patients of all ages.
• Perform physical examinations, diagnose and treat acute and chronic medical conditions, and develop appropriate treatment plans.
• Prescribe and manage medications for Medication-Assisted Treatment (MAT), as appropriate and in accordance with applicable federal and state requirements.
• Order, interpret, and communicate laboratory and diagnostic test results.
• Deliver preventive care, including routine screenings, immunizations, and health education.
• Coordinate care with specialists, behavioral health providers, and community resources.
• Participate in quality improvement initiatives and support organizational performance measures.
• Promote a welcoming, respectful, and culturally responsive environment for all patients.

We are seeking a Family Nurse Practitioner to join our integrated care team to provide comprehensive primary care services, including health assessments, diagnosis and treatment of acute and chronic illnesses, preventive care, health education, and care coordination.
