NorthShore Health Centers is seeking a Physician to provide full-scope Family Medicine in Chesterton, IN. You will diagnose, treat, and coordinate care for patients of all ages within the PCMH model and NSHC credentialing guidelines.

You will collaborate with the patient care team, counsel patients, and develop care plans while meeting credentialing requirements for NSHC and associated hospitals. Indiana licensure and DEA/CSR credentials are required, with two years' experience preferred.

Qualifications

• Must be licensed physician in Indiana.
• CSR license in Indiana.
• DEA license.
• Two years of healthcare experience preferred.
• Graduate of accredited medical school.

Responsabilités

• Provide full scope of primary care services including diagnosis, treatment and care coordination.
• Prescribe medications in accordance with Indiana law and professional guidelines.
• Coordinate care with the patient care team and specialists as needed.
• Maintain credentialing requirements for NSHC and affiliated hospitals.
• Participate in quality assurance, peer review and staff meetings.
