Cornerstone Care in Clairton, PA is seeking a Family Practice Physician to join our FQHC team. The physician will diagnose and treat patients across preventive and chronic care, oversee care with mid-level practitioners, and ensure accurate documentation.

Qualifications

• Graduate of an accredited medical school with MD/DO degree.
• Residency in Family Practice with board certification.
• Current license to practice medicine in Pennsylvania.

Responsibilities

• Care for patients with chronic illnesses and preventive health
• Conduct physical exams, take histories, and order/interpret tests
• Provide health literacy appropriate disease management education
• Collaborate with care team to set treatment goals
• Ensure complete and accurate documentation in the EHR
