Qualifications

• Medical Degree (MD or DO) from an accredited medical institution.
• Board-eligible or certified in Family Medicine.
• Valid IL medical license or ability to obtain licensure.

Responsibilities

• Provide patient-centered care to individuals and families across all ages.
• Manage a broad spectrum of health issues of acute and chronic conditions while promoting wellness and preventative care.
• Collaborate with multidisciplinary teams and other healthcare professionals.
• Maintain a healthy work-life balance in a supportive environment.

Education
