This full-time position is based in a Federally Qualified Health Center where you will provide comprehensive outpatient primary care alongside behavioral health providers, nurses, and care coordinators.

Qualifications

• Active license in Kansas (or eligible).
• Ability to provide care to patients of all ages.
• Excellent communication and teamwork skills.

Responsibilities

• Provide direct outpatient primary care services to patients of all ages
• Collaborate with behavioral health and nursing teams in an integrated care model
• Use evidence-based protocols to manage acute and chronic conditions
• Foster long-term relationships with patients and support lifestyle changes
• Participate in quality improvement and risk management activities
• Document care using our electronic health record system
