Qualifications

• Outpatient family medicine physician role.

• Shared call 1:20 or better.

Responsibilities

• Provide outpatient Family Medicine care in a clinic setting.
• Collaborate with RN co-visits and integrated clinical services (PharmD, Behavioral Health, RD).
• Participate in patient-centered medical home initiatives.
