• Collaborate with behavioral health and nursing teams in an integrated care model.
• Use evidence-based protocols to manage acute and chronic conditions.
• Foster long-term relationships with patients and support lifestyle changes.
• Participate in quality improvement and risk management activities.
• Document care using the electronic health record system.
