Hamilton Health Center, a mission-driven Federally Qualified Health Center (FQHC), is seeking a Family Medicine Physician to deliver high-quality, patient-centered care in Millersburg and Newport, PA. This full-time role provides outpatient primary care across the lifespan within a team-based, integrated care model.

The position emphasizes whole-person care with behavioral health, social services, and care coordination support, enabling meaningful, outcomes-driven care.

Qualifications

• MD or DO degree required.
• Board certification or eligibility in Family Medicine.
• Active Pennsylvania medical license or ability to obtain one.
• Strong communication and interpersonal skills.
• Proficiency with EMR systems.

Responsibilities

• Provide comprehensive primary care services, including preventive care, chronic disease management, and acute care.
• Perform full physical examinations and annual health assessments.
• Conduct basic gynecologic exams as part of routine care for adult female patients.
• Diagnose and manage medical conditions, coordinating care as needed.
• Maintain accurate, timely, and compliant documentation in the EMR system.
• Collaborate with behavioral health, care management, and support staff to deliver integrated care.
• Participate in quality improvement initiatives, peer review, and case management discussions.
• Contribute to on-call coverage per provider schedule.
• Attend staff meetings, trainings, and required in-service sessions.
• Provide supervision to Nurse Practitioners and Physician Assistants, as applicable.
• Ensure compliance with HIPAA, OSHA, and organizational standards.
