The clinic features a patient-centered medical home model and multi-disciplinary team including physicians, advanced practice providers, psychologist, case manager, and clinical pharmacist.

This full-time position offers a $50,000 signing bonus, up to $80,000 in educational loan repayment,

Position Details

• Full-time position, 1.0 FTE (flexible to hire 0.9 FTE to 1.0 FTE)
• Schedule can be either 4, 4.5, or 5 days per week in clinic
• Outpatient care only with minimal phone call

Compensation & Benefits

• $50,000 signing bonus and up to $80,000 in educational loan repayment funds
• Guaranteed base salary, incentives, and relocation assistance available
